Dear Parents and Players,

We would like to officially welcome you to Benet Academy’ s 2012 Redwing Lacrosse Club
season and thank you for your continued interest in and support of the Redwing Lacrosse Club.

Plans for the 2012 season began shortly after our teams finished their 2011 season last May against
some stiff competion which included the Chicago Catholic League. We are fortunate that our schedule
not only offers great competition exposing our teams to many of the local lacrosse programs, but also
to teams that share our values and faith as well.

In an effort to keep our registration process streamlined, all registration forms are being
distributed via email and will also be available on our website www.benetlax.org . The forms are
as follows:

• Welcome Letter

• Player Registration & Medical Form*

• Release and Waiver Form*

• IHSA Physical*

• Payment options*

- Uniform Deposit Form*

 

* These required forms need to be printed (in color if possible!) and completed in their
entirety. We have kept the Registration Form in a Microsoft Word format so please type the information,

if possible. All players, new and returning, are required to return the completed forms to the
registrar at the mandatory 2012 registration meeting on November 14th, 7:00 PM in St. Jude Commons. 

(If you participated in a fall sport, just write across the IHSA physical form that the

 include personal equipment or off season training costs. Similar to previous years, we will be offering two
payment options:

• Option 1 is to pay the fee in full on registration night.
• Option 2 will be to spread the cost into three payments of $225, $225 and $200.

Note: all three checks will be collected on the registration night, but the second and third
installment checks will be postdated and held until the due date, similar to the Uniform Deposit.

Redwing Lacrosse Club will send out a reminder prior to the deposit dates listed above.
